*Membership plan eligibility is determined following a comprehensive examination and periodontal assessment. Patients diagnosed with periodontal disease may require enrollment in the Lifetime Smile Plan rather than the Healthy Smile Plan. Our team will review all findings and recommend the membership option most appropriate for your oral health needs.
